Job description
Hartford Healthcare is seeking a Palliative Care provider to deliver compassionate, patient-centered care for individuals with serious illness across inpatient and outpatient settings. In this role, you will conduct comprehensive assessments, manage complex symptoms, and facilitate goals-of-care and end-of-life discussions with patients and families. You will collaborate closely with an interdisciplinary team, including physicians, nurses, social workers, and chaplains, to create individualized care plans.
