Job description
Empatia, part of Gentiva, is seeking a compassionate Palliative Care Nurse Practitioner to deliver advanced home-based palliative care in collaboration with the Palliative Medical Director.
You will perform comprehensive assessments, develop individualized care plans, prescribe medications as allowed, and participate in evening/weekend call rotations while maintaining documentation and coordinating with the care team to support patients and families.
