You will assess symptoms, develop individualized treatment plans, support goals-of-care discussions, and collaborate closely with patients, families, facility staff, physicians, and interdisciplinary care teams.Key ResponsibilitiesProvide compassionate, person-centered palliative care using evidence-based practices.Conduct comprehensive assessments and develop individualized treatment plans.Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.Support goals-of-care discussions with patients and families.Collaborate with physicians, facility staff, and interdisciplinary care teams.Educate patients, families, and facility staff regarding palliative care and treatment plans.Complete timely and accurate clinical documentation.Maintain compliance with facility, state, and federal regulations.Join a Team That Supports YouIf you're an NP looking to provide meaningful, patient-centered care with flexibility, autonomy, and strong clinical support, we'd love to hear from you.Apply today.
