Department:
300 - PPP - Clinic - Palliative Care
Position Description:
Responsible for performing and documenting psychosocial assesments, coordinates educational and outreach services relevant to the social services, and provides liaison function between system-wide outpatient clinics.
Qualifications:
1.Master's degree in Social Work from an accredited institution
2.Experience in Palliative Care or hospice setting preferred
3.Licensed Clinical Social Worker (LCSW) certification required
4.Strong understanding of end-of-life care and ethical issues in Palliative Care.
5.Minimum of 2 years' experience in a healthcare setting
6.Familiarity with Epic, ICD-10, and CPT coding preferred.
