Job description
St. Vincent’s Medical Center seeks a Parish Registered Nurse in Community Health to coordinate faith-based, community-focused nursing care for parish members in Bridgeport, CT. The RN will conduct health assessments, chronic disease education, care coordination, and referrals; organize screenings, wellness programs, and support groups; and collaborate with clergy, social services, and hospital teams to address spiritual, emotional, and physical needs.
