Job description
St. Vincent’s Medical Center seeks a Parish Registered Nurse in Community Health to coordinate faith-based, community-focused nursing care for parish members in Bridgeport, CT. The RN will conduct health assessments, chronic disease education, care coordination, and referrals; organize screenings, wellness programs, and support groups; and collaborate with clergy, social services, and hospital teams to address spiritual, emotional, and physical needs. This role supports diverse populations, promotes prevention and safe transitions of care, documents outcomes, and participates in continuous learning to improve community health and patient-centered care.
