Job description
InterCare Community Health Network in Holland, MI, seeks a part-time Behavioral Health Consultant (20+ hours/week) to support behavioral health integration in primary care within a Federally Qualified Health Center setting.
The role includes consultation to providers, brief screenings, short-term interventions, and crisis management, with referrals and team-based care. Masters in Social Work with LLMSW/LMSW preferred; LPC considered; strong commitment to access to care.
