Job description
St. Gregory Recovery Center in Bayard, Iowa is seeking a part-time Case Manager to support multiple units by coordinating care across providers, developing ISP, and leading discharge planning.
The role operates onsite with the treatment team to ensure clients receive individualized services aligned with their treatment plans. Responsibilities include maintaining clinical records, coordinating referrals and entitlements, engaging clients and families, and applying crisis intervention when needed.
