Job description
Elara Caring is hiring a part-time Medical Social Worker to support home health patients and families in Portland and southern Maine communities. You will assess psychosocial, financial, and environmental needs, provide counseling, and coordinate resources to help patients remain safely at home.
You will work independently in the community with support from an interdisciplinary team, travel 50% of the time, and contribute to care planning and documentation while advancing your professional
