Optum Home & Community Care’s HouseCalls program seeks Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants) to perform in-home wellness assessments for Medicare Advantage members in a flexible, field-based role. This non-prescribing position emphasizes assessment, education, and coordination with primary care providers to close gaps in preventive care and improve outcomes.
Visit patients in their homes 24 hours per week with scheduling that fits between 8:00 AM and 8:00 PM;
