Job description
21st Century Home Health Services is seeking a Part Time/Weekend Physical Therapist Case Manager to provide in-home PT services across a designated territory including Yuba City. You will evaluate patients at home, develop care plans, and work with PTAs to execute the plan, focusing on restoring mobility and independence.
Requirements include an unrestricted CA PT license, BLS/CPR, and a valid CA driver’s license.
