Advanced Health Care is seeking a Home Health RN Case Manager to coordinate skilled nursing care in patients' homes. You will assess needs, develop care plans, administer treatments, and educate patients and families, while collaborating with an interdisciplinary team.
Responsibilities include supervising home health aides, ensuring regulatory compliance, and evaluating outcomes to adjust plans. Part-time, PRN with flexible weekday/weekend shifts and mileage reimbursement offered.
