Job description
Athena Hospice of Rhode Island is seeking a part-time RN Case Manager to join our dedicated team. You will plan, organize, and direct the hospice care plan for assigned patients under the clinical director, with a competitive hourly rate and weekly pay; travel throughout Rhode Island is required.
Responsibilities include conducting initial and ongoing assessments, developing and revising care plans, supervising hospice aides, and delivering professional nursing care using the nursing process.
