Job description
Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP/PA) to provide in-home health evaluations in Fergus Falls, MN and surrounding areas. You will conduct 45-minute visits, review medical history, perform a physical evaluation, and coordinate with health plans for indicated tests. Travel locally and by license as needed; virtual visits also when required.
The position offers part-time hours (approx. 29.5 per week) with select benefits and malpractice coverage.
