Job description
Signify Health, part of CVS Health, seeks a Part-time Nurse Practitioner or Physician Assistant to perform in-home health evaluations in Albany and Fulton counties, NY. Clinicians will connect with health plan members at home, gather histories, perform brief exams, and coordinate care as needed.
