Job description
Optum Home & Community Care, part of the Optum family, is seeking a Nurse Practitioner or Physician Assistant for the Senior Community Care (SCC) program in Elkhart, IN, St. Joseph, Kosciusko, and Marshall counties.
This part-time role includes a $10,000 sign-on bonus for external candidates, no on-call, no weekends, and no holidays, with benefits for 20 hours per week. You will join a team delivering integrated care to high-risk members in nursing homes, senior housing, and assisted living
