UnitedHealth Group seeks a HouseCalls Advanced Practice Clinician (NP or PA) for in-home Medicare Advantage care. This part-time 24-hour role focuses on comprehensive assessments, education, and care coordination with primary care providers.

You’ll work in patients’ homes, facilitating access to preventive services and addressing social determinants of health. The position emphasizes flexibility in scheduling (8am-8pm across 7 days) with no on-call, weekends, or holidays required.
