Job description
Greater Good Health in Salem, OR seeks a Part-Time Nurse Practitioner to manage outpatient and home-based care for senior patients. You will partner with clinics as an embedded care team member to prevent hospitalizations, optimize chronic disease management, and support holistic patient plans.
Responsibilities include wellness exams, care coordination, HIPAA compliance, and collaboration with social workers and case managers. Travel within Salem area 5 days/week is required.
