An exceptional opportunity awaits a dedicated Nurse Practitioner in Berea, KY, to join a unique facility-based long-term care model. You will serve as a primary care provider for elderly residents, managing their health needs in a familiar and consistent setting. The day-to-day clinical environment is collaborative, working closely with facility staff to ensure seamless care coordination and optimal patient outcomes. We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family medicine. The ideal candidate will be comfortable managing complex chronic conditions common in the elderly population and possess strong clinical judgment. A collaborative spirit and excellent communication skills are essential for thriving within our interdisciplinary team. ## Key Responsibilities - Provide primary care to long-term care residents in skilled nursing facilities - Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment - Conduct advanced care planning and manage chronic disease conditions - Prescribe medications including controlled substances in accordance with Kentucky law - Communicate with nursing home staff and collaborate with attending physicians on care plans ## Qualifications - Current, unrestricted Nurse Practitioner license in Kentucky (required) - National board certification (AANP or ANCC) - Active DEA licensure - Clinical background in adult, family, or geriatric specialties - Experience in long-term care or skilled nursing facility setting preferred - Proficiency with electronic medical records (EMR) ## Benefits - Continuing education credits, recertification reimbursement, malpractice insurance coverage, bonus pay Schedule - Part Time-based schedule serving skilled nursing facilities in Berea, KY Location Berea, KY 40403 Berea, Kentucky, offers a charming small-town atmosphere nestled in the foothills of the Appalachian Mountains, providing a peaceful lifestyle with a strong sense of community.
