Job description
Assesses residents’ health and ongoing needs, develops and updates individualized care plans, and coordinates with physicians, families, and interdisciplinary team members. Administers medications, treatments, and IV fluids; documents assessments and interventions; monitors changes in condition; and supports residents with daily care in a long-term care setting.
