Job description
Home Care Network, Inc. is seeking a skilled RN Case Manager/Recertification Nurse to coordinate care, reassess medical necessity, and supervise a care team across Brown County and surrounding Southern Ohio areas on a part-time schedule.
You will perform SOC, admissions, recertifications, and ROC, with documentation and cross-functional communication to physicians and families; travel is required within a 60–120 minute radius.
