Job description
GLIDE is a Tenderloin-based social services organization in San Francisco seeking a Transitional Aged Youth Health & Wellness Case Manager. This part-time role (up to 16 hours/week) handles intake, assessments, counseling and coordination of housing, education, health, and community supports.
