Job description
Join St. Vincent’s Medical Center, part of Hartford HealthCare, as a Case Coordinator supporting compassionate, patient-centered care across Fairfield County. Coordinate services with multidisciplinary clinical teams, help patients and families navigate appointments, referrals, follow-up care, and community resources, and identify barriers that may affect access or outcomes.
