Job description
Total Health Care in Baltimore, MD is seeking a Patient Engagement Worker to help patients and the surrounding community navigate healthcare systems and improve health outcomes. The PEW links patients with the health center, supports value-based care, and coordinates appointments, referrals, and transportation while maintaining strong relationships with patients and care teams.
