Job Summary
The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

Able to operate a computer and knowledge of Microsoft Office and be proficient in the use of electronic health record systems.
Fluent written and verbal skills in English and Spanish strongly preferred

Job Requirements
Bachelor's degree in health-related field. Nursing or Social work degree preferred.
Minimum one (1) years’ experience in clinical care setting
Community Health Worker Certification or be able to obtain within first three months to six months of employment.
Experience working with diverse populations, including individuals with behavioral health or substance use needs, preferred
