Job description
Community Care Physicians is seeking a Patient Support Specialist to deliver exceptional, patient-centered service across our multispecialty practices in New York’s Capital Region. In this role, you will be a key liaison for patients, handling scheduling, check-in/check-out, insurance verification, portal assistance, and non-clinical inquiries. You will collaborate closely with clinicians, nurses, and administrative staff to coordinate care, resolve concerns, and ensure accurate EHR documentation.
