Job description

Community Care Physicians seeks a Patient Support Specialist to deliver exceptional, patient-centered service across our Capital Region practices. In this role, you will be a key first point of contact, handling calls, scheduling and confirming appointments, verifying insurance, and assisting with referrals and records requests. You will guide patients through check-in, check-out, and portal use while maintaining accurate information in our electronic systems. Working closely with clinicians and staff, you’ll help coordinate care, resolve issues, and support a welcoming, team-focused environment with opportunities to grow across our large, physician-owned medical group.
