Eleos Home Care Caregiver PositionAt Eleos Home Care, we are always looking for compassionate and dedicated individuals to join our team of caregivers. Whether you are an experienced caregiver or looking to start a career in healthcare, we welcome you to join our team.Duties of PositionProvides personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse.Tasks to be performed by a caregiver must be assigned by and performed under the supervision of an RN who will be responsible for the client care/services provided by the caregiver.Under no circumstances may a caregiver be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures.Position ResponsibilitiesFollows the plan of care/service to provide safe, competent care/service to the client.Performs the following tasks as assigned by the RN: Assist with the activities of daily livingMedication remindersTaking and recording vital signsRecording and reporting to the supervisor any changes regarding the client's condition, behavior or appearanceDocumenting the services delivered in the client's recordHelps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.Plans and prepares nutritious meals.Assists the client with ambulation as approved.Encourages the client to become as independent as possible according to the care plan.Attempts to promote client's mental alertness through involvement in activities of interest.Gives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmits trust and confidentiality.Completes at least 12 hours of in-service training and education annually.Performs routine housekeeping tasks as related to a safe and comfortable environment for the client.Prepares a visit report on the day the service/care was provided.Confirms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated.Works with personnel of other community agencies involved in the client's care.Confers with the Supervisor about client's status and care, participates in the planning of service.Promptly report any change in client condition to the Supervisor.Works within the caregiver's scope of practice, as directed by a Registered Nurse and report in writing to the Supervising Nurse.Job ConditionsThe ability to drive and the ability to access clients' homes which may not be routinely wheelchair accessible are required.Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds.Must be able to communicate clearly, both verbally and in writing.Equipment OperationUse of BP cuff, thermometer, stethoscope, and hand washing materials.Company InformationHas access to all client service records which may be discussed with the supervisor.QualificationsBe able to speak, read and write English.Have a valid driver's license, car insurance, and reliable form of transportation.Pass a criminal background check.Availability to work different shifts, including nights and/or weekends.Eleos Home Care provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
