We are currently seeking FT and PT HealthCare Heroes to join our Constantly Caring team.QualificationsCurrent Home Health Aide Certificate in the State of New YorkResponsibilitiesAssist with patients Personal Care needs: personal hygiene, bathing, dressing/undressing, and ambulation together with any household task (eg making beds, changing linen, laundry, light housekeeping, etc) essential to the patients' health in accordance with the patient's individualized Plan of Care.Plan/Prepared nutritious meals and assist in feeding patients as directed. Document services provided in accurate and timely manner in accordance with company policy and personal care plan.Maintain Confidentiality at all timesReport any changes in the patient's condition or incidents to RN SupervisorCarry out other dutiesAccompany patients to their doctor visitsBenefitsPaid HolidaysDirect Deposit weeklyFlex Spending (Cell phone, Transportation, medical)PTONYC Sick Time401K
