Job description
Nemours Children's Health is seeking an Advanced Practice Provider to join the Advanced Care at Home (CACaH) program in Jacksonville, FL. The APP will collaborate with physicians to round on inpatient units, enroll eligible patients in the program, and deliver follow-up care through Telehealth after discharge.
