Job description
Nemours Children’s Health System is seeking an Advanced Practice Provider to join the Advanced Care at Home program in Jacksonville, FL. The APP will participate on a multidisciplinary team, delivering comprehensive pediatric care as patients transition from hospital to home.
The role involves rounding with a physician, enrolling patients, and providing follow-up Telehealth visits after discharge. Robust collaboration with nursing staff and resource coordination for families is required.
