Job description
Rutgers University is seeking a Health Care Case Manager (Registered Nurse) for the Morris East Local Office in Morris Plains, NJ, to support the DCF/CP&P–Child Health program. The HCCM collaborates with the health team to enroll children with a medical home, review health histories, and coordinate care plans across providers.
