Job description
CareGivers America – NEPA is seeking a full-time RN Case Manager to support pediatric patients and families across Luzerne, Wyoming, and Sullivan County. You will coordinate care, develop pediatric plans with physicians, supervise LPNs, and ensure accurate documentation, all while traveling to patients’ homes and working within a supportive team.
The role offers a Monday–Friday schedule, 40 hours, with a company vehicle provided and travel time included.
