Job description
Anchor Health is seeking a Pediatric Hospice RN Case Manager to provide professional nursing care for pediatric patients. You will assess, develop, implement and evaluate care plans with input from an interdisciplinary team, ensuring effective communication with patients, families, and providers.
Travel for patient visits is required, with occasional educational travel. The role requires an Associates degree or higher, CA RN license, CA driver’s license, CPR/BLS, and pediatric hospice
