Job description
Sanford Health is seeking a pediatric care manager nurse to use the nursing process to deliver individualized patient care across hospital and clinic settings. You will collect and relay information to providers and coordinate care across the continuum in multiple locations.
You will develop education plans, implement care plans, and lead population health initiatives while adhering to clinical guidelines and standards of nursing practice. A strong RN background and BLS are required.
