Job description
Home Heart Florida is seeking a Start of Care (SOC) Nurse to perform initial pediatric home health evaluations, establish care plans, and set the tone for a safe, high-quality home care experience.
You will conduct start of care assessments in patients' homes, develop plans based on physician orders, educate families on care needs, equipment, medications, and safety, and document thoroughly for the clinical team.
