Job description
Providence Health and Services is seeking a Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This Per Diem role involves 8-hour Day shifts to coordinate patient care and transitions.
The Case Manager will monitor quality indicators and liaise with patients to ensure smooth progression through the care continuum. Required California RN License and 2 years acute care experience; preferred Bachelor's in Nursing, CCM, and 3 years in community health or case management.
