Sauk Prairie Healthcare is seeking a Clinical Pharmacist (Per Diem) to join our team across inpatient, outpatient, and clinic settings. The role focuses on reviewing orders, providing pharmacotherapy recommendations, and educating staff.
Responsibilities include therapeutic drug monitoring, TPN oversight, and Code Blue participation, with a minimum commitment of 3 shifts per month. Strong collaboration within interdisciplinary teams is essential.
