Overview
nRelief Community Support Professionals are able to build flexible schedules that fit their availability; covering shifts as needed.
nPaid Mileage! All Hours Available!
nLooking for fun, creative and flexible staff to provide community based support services for individuals with developmental disabilities of all ages. Community Support Professionals work one on one with individuals on their caseload to implement unique services, to explore and build skills which support increased independence, inclusion, and community engagement. In terms of work locations listed above, you have the option of choosing the towns you prefer with a flexible daytime work schedule. This position can be full or part time in Cortland County. This position requires that you have your own transportation. We also have a generous reimbursement for mileage, $.76 per mile.
nThose working 10 hours or more rate is $20.00 per hour. If working fewer than 10 scheduled hours per week you will earn $19.00 per hour.
nResponsibilities
nnnResponsible for the health and safety of the individuals supported
n
nnProvide individualized services to people with disabilities and their families which may include: respite for families, teaching, modeling, and supporting skill development in a variety of areas at home and in the community
n
nnParticipate as an active member of the person’s team providing ideas, input, and advocacy for the person related to his or her Staff Action Plan
n
nnFollow objectives as described in the person’s Staff Action Plan and as discussed with supervisor
n
nnAttend training to improve services provided
n
nnComplete job requirements (training, PPD, etc ) within appropriate time frames
n
nnReport any potential incident to the designated Family Support Coordinator/Supervisor immediately, or the day that it occurred.

nnAccurately enter time and services into Racker’s electronic health record, eVero, documenting service times, supports provided, and mileage according to program and regulatory requirements (e.g., contemporaneously documenting service provision)
n
nnUse of smartphone for documentation purposes required . Ownership/usage of personal smartphone is strongly preferred
n
nnCommunicate regularly with supervisor regarding service provision
n
nnAttend scheduled supervisory meetings regularly
n
nnUphold standards of professionalism with regard to behavior and dress
n
nnAssume responsibility for related duties as required or assigned
n
n nQualifications
nnnHigh school diploma or GED
n
nnAbility to meet the agency's approved driver criteria (3 years’ experience with a driver’s license and no more than 4 points)
