Per Diem Home Care Nurse (RN/LPN) job at Abernathy Home Care. Stoneham, MA.
About Us:We are a compassionate, patient-centered home care agency proudly serving the Stoneham, MA, area. Our mission is to provide high-quality, personalized care in the comfort of our clients' homes. We seek experienced and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our per diem team. Position Summary:As a per diem home care nurse, you will oversee our clients' care in their homes. This is an excellent opportunity for nurses seeking flexibility, autonomy, and meaningful one-on-one patient care interaction. Responsibilities:
Perform initial and ongoing nursing assessments to determine client needs.
Develop, implement, and revise personalized care plans
Provide education and health coaching to clients and caregivers on disease management, medications, and home safety
Coordinate services and follow-ups with ASAP's ( Aging Service Access Point) as well as family members
Supervise and evaluate home health aides and ensure plan-of-care compliance
Maintain accurate and timely clinical documentation following regulatory and agency standards
Participate in quality assurance activities and support compliance with state and home health regulations
Requirements:
At least 18 years of age
Current MA RN or LPN license in good standing
A driver’s license in good standing.
Reliable transportation
Must be able to pass criminal background and professional reference checks
Strong communication skills, both verbal and written, to facilitate effective interactions with clients
Ability to respect the confidentiality of patients and families
Minimum of 1 year of clinical nursing experience (home care experience preferred) * CPR certification
