About This Role
The Per Diem Home Health Clinical Admission Specialist RN at Mount Sinai is responsible for identifying, evaluating, and enrolling patients into Mount Sinai at Home programs. This role is ideal for RNs who thrive in collaborative environments and have a strong clinical background.

What You'll Do
Identify and evaluate patients for appropriateness for Mount Sinai at Home programs.
Facilitate coordination and communication between all members of the care team to coordinate admissions.
Ensure knowledge and acceptance of Mount Sinai at Home with patients, family members, and ED/hospital teams.
Maintain all required documentation.
Interact with patients and teams to coordinate services ordered for physicians.
Clearly communicate transition plans and information to all members of the care team.
Build and maintain collaborative professional working relationships with physicians, nurses, and the community.
Screen and question patients and families to ensure safe home environments.
Ensure that patients have access to appropriate services to meet their provider-directed care plan admission needs.
Comply with policies, procedures, and regulatory mandates.
Requirements
Education
Bachelor’s Degree in Nursing (BSN) required, Associate’s Degree in Nursing (ADN) acceptable if approved by the CNO.
Experience
1+ year experience working as an RN required.
Preferred: 2+ years experience as an RN, experience in Emergency Departments, Intensive Care Unit, or Med Surg.
Licenses & Certifications
Current RN License in New York State.
Basic Life Saver (BCLS) certification required.
