Job description
Per Diem Home Health Nurse Practitioner in Aurora, IN
Step into a vital role delivering healthcare directly to patients in their homes, focusing on Medicare Advantage plan members throughout Dearborn County. This per diem position empowers experienced nurse practitioners to have a meaningful impact by conducting comprehensive home health risk assessments in diverse communities such as Lawrenceburg, Greendale, and Aurora.
Schedule:
Flexible shifts available Monday through Sunday
Create your own work schedule
Commit to weekly or bi-weekly minimum hourly requirements
Responsibilities:
Perform annual home health risk assessments for Medicare Advantage members in patients homes
Gather, review, and evaluate prior/current medical history, chronic conditions, and medication records
Educate members on their conditions and prescribed treatments
Identify and address gaps in care, providing recommendations as needed
No treatment planning or prescribing required for this position
Qualifications:
Active Nurse Practitioner license in Indiana
Current CPR certification (BLS required)
Board Certification through AANP or ANCC
MSN from an accredited nursing school
At least 6 months of recent experience working as a nurse practitioner preferred
This position offers the opportunity to be the clinical face of home health risk assessments, foster patient engagement, and support continuity of care without the responsibilities of ongoing case management. Compensation for each evaluation ranges from $80$90, with additional mileage reimbursement and coverage for administrative time.
