Job description
Per Diem Home Health Nurse Practitioner in Canton, NY
Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments serving Medicare Advantage members throughout St. Lawrence County. This dynamic position allows you to provide crucial healthcare assessments in home settings, with visits typically lasting around 45 minutes. Each assessment may range from annual wellness visits to post-discharge evaluations, with flexibility to serve clients in areas including Canton, Potsdam, Massena, and Ogdensburg.
Schedule:
Flexible shifts available any day, Monday through Sunday
Self-scheduling to accommodate your lifestyle and commitments
Choose your own minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Conduct comprehensive home health risk assessments for Medicare Advantage members
Review and evaluate patients current and prior medical history, ongoing conditions, and medication regimen
Offer education to patients regarding their medical conditions and prescribed medications
Identify gaps in care and deliver recommendations to support optimal health outcomes
No prescribing or treatment plan creation requiredfocus remains on holistic assessment and education
Qualifications:
Active Nurse Practitioner license in New York
Board Certification through AANP or ANCC
Current CPR certification (BLS)
MSN degree earned from an accredited nursing school
At least six months of current Nurse Practitioner experience highly preferred
Compensation & Benefits:
Earn $80$90 per completed evaluation
Compensation includes a mileage stipend and a rate for administrative time
