Job description
Per Diem Home Health Nurse Practitioner in Machesney Park, IL
Delivering compassionate, individualized patient care, you will take on a vital role conducting home health risk assessments for Medicare Advantage plan members in Winnebago County, Illinois. This per diem opportunity allows you to exercise clinical expertise while enjoying true schedule flexibility, as you create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly commitment. Locations may include Rockford, Loves Park, and Machesney Park.
Key Responsibilities:
Perform annual home health risk assessments for Medicare Advantage health plan members
Review and document prior/current medical history, chronic conditions, and medication usage
Provide patient education on medical conditions and medication management
Identify care gaps and offer clear recommendations to help resolve them
Visits are typically 45 minutes and may be annual health assessments or post-discharge assessments
No prescribing or development of treatment plans required
Schedule:
Flexible scheduling between Monday and Sunday
Set your own shifts to match your availability
Minimum weekly or bi-weekly hourly commitment
Qualifications:
Active Nurse Practitioner License in Illinois
Board certification via AANP or ANCC
Current BLS/CPR certification
MSN from an accredited nursing program
At least 6 months of current Nurse Practitioner experience preferred
