Job description
Per Diem Home Health Nurse Practitioner (NP) - Clinton, Indiana
Contribute your clinical expertise as a Nurse Practitioner delivering impactful home health risk assessments to Medicare Advantage plan members throughout Vigo County, IN, including Terre Haute, West Terre Haute, and Seelyville. This per diem position offers a high degree of flexibility, allowing you to design a schedule that aligns with your lifestyle and commitments.
Schedule:
Flexible shifts available Monday through Sunday
Customize your own schedule
Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Conduct in-person annual and post-discharge home health risk assessments
Review and evaluate each patient’s medical history, conditions, and medication usage
Educate members on their medical conditions and prescribed medications
Identify care gaps and provide actionable recommendations
No responsibility for writing treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner License in Indiana
Current BLS/CPR certification
Board Certification through AANP or ANCC
At least 6 months of recent NP experience is preferred
MSN from an accredited nursing program
