Job description
Per Diem Home Health Nurse Practitioner (NP) - Rock Falls, IL
Advance your clinical expertise by providing meaningful care in a per diem home health role. As a Nurse Practitioner specializing in home health, you will perform comprehensive risk assessments for Medicare Advantage plan members across Whiteside County. Each visit typically lasts about 45 minutes, where you’ll have an opportunity to make a vital impact whether it’s an annual assessment or a post-discharge visit. Assignments may occur in Sterling, Rock Falls, Morrison, or other locations in the county, giving you the flexibility to serve clients close to home.
Schedule:
Flexible shifts available Monday through Sunday
Design your own schedulework when it suits you
Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Complete thorough annual home health risk assessments for Medicare Advantage members
Review prior and current medical history, conditions, and medication usage
Educate members on their diagnoses, conditions, and medications
Identify gaps in care and make practical recommendations to close them
Focus exclusively on assessment and educationno prescribing or treatment plan creation required
Qualifications:
Active Nurse Practitioner license in Illinois
Current BLS/CPR certification
Board certification via AANP or ANCC
Minimum of 6 months recent experience as a Nurse Practitioner preferred
Master of Science in Nursing (MSN) from an accredited institution
You’ll receive payment at $80$90 per evaluation, with an additional stipend for mileage and compensation for administrative time.
