Job description
Advance your career in a dynamic per diem role focused on Home Health risk assessments for Medicare Advantage plan members in Shawnee County, Kansas. As a Nurse Practitioner, you will conduct comprehensive assessments in members homes, spending approximately 45 minutes per visit. Assignments may include annual health assessments or post-discharge evaluations in locations such as Topeka, Silver Lake, and Auburn.
Schedule:
Flexible shifts available Monday through Sunday
Ability to customize your own schedule
Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
Complete annual home health risk assessments for Medicare Advantage plan members
Gather and evaluate medical history, existing conditions, and medication usage
Educate patients on their medical conditions and medication regimens
Identify gaps in care, recommending solutions to enhance patient outcomes
No treatment planning or medication prescribing required
Qualifications:
Active Nurse Practitioner license in Kansas
Board Certification through AANP or ANCC
Masters degree in Nursing (MSN) from an accredited institution
Current CPR (BLS) certification
At least 6 months of recent experience as a Nurse Practitioner preferred
