Job description
CenterWell Home Health seeks a licensed Physical Therapist to help patients regain strength, mobility, and independence in their homes. You will evaluate, plan, and administer prescribed therapy to maximize function and quality of life.
You will collaborate with a care team, supervise PTAs and home health aides, document progress, and communicate with physicians and family. A state license and CPR certification are required, with a valid driver’s license for home visits.
