Job description
Heartland Home Health in West Branch, MI seeks a Per Diem Physical Therapist to provide home health coverage for patients in the local area. The role requires a Michigan PT license (or eligibility), CPR, and strong assessment and communication skills. Coverage is 1–2 days per week with no on-call or weekends.
The PT will deliver skilled therapy per physician plans, educate patients and caregivers, and collaborate with nurses and other therapists to maximize independence and safety at home.
