Job description
Are you a compassionate and skilled Nurse Practitioner looking to make a meaningful impact in the lives of children and young adults? We are seeking a dedicated Per Diem NP to provide home-based care to pediatric and young adult patients experiencing pain and medical challenges.
nLocation : Brooklyn, Queens, Bronx, Manhattan, Staten Island, and Westchester, NY
nType : Per Diem, 1099 Independent Contractor
nSpecialty : Providing compassionate nursing care to children and young adults in a home setting, focusing on pain management, symptom relief, and overall well-being.
nResponsibilities
nnPerform comprehensive in-home assessments and develop personalized care plans
nProvide pain management interventions and monitor patient symptoms
nEducate families and caregivers on medication administration, symptom monitoring, and care techniques
nCollaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure holistic patient care
nMaintain accurate clinical documentation and adhere to all regulatory and professional standards
n nQualifications
nnActive Nurse Practitioner (NP) license in New York State in good standing
nMinimum of 1 year working with a medically fragile population
nExperience in pain management
nStrong communication and interpersonal skills to provide emotional support to patients and families
nAbility to work independently and travel to patient homes as needed

nn$170 per hour
